  1. Dear Dr. Rosenberg, After a very successful breast reduction operation performed 18 months ago, I recently had a "suture" surfacing around the areola scar and this area had become infected. Not knowing that the suture was a "purse-string suture" the general surgeon that I went to yesteday pulled it out entirely. (Unfortunately, I live out of state where the procedure was performed.) Now understanding that the suture had an esthetic purpose to keep the areola area tight, should this/can this be reinstated? - anonymous from California

I see no reason to replace this pursestring suture after this period of time. Healing and maturation of the tissue is now complete and it is highly unlikely that your areola will expand. In any case, should you notice a gradual increase of areolar size over time, a minor revision could be easily performed. Why go through additional surgery, if it is probably not necessary?   « less
  2. I had fat injections in my cheeks, naso-labial & lip area one week ago. I expected swelling, but not this much. I am concerned because the right side swelling is much more pronounced than on the left. Is there a chance it will stay that way? I've been sleeping on my back with my face elevated, and I haven't been touching my face at all. Thank you for your time and attenion - Yvonne from Tampa, Florida

Jose, At four to five weeks, scarring in any surgery it at its maximum. Fullness of the your lower abdomen after drainage of a persistent seroma would account for the fullness. Time should allow this to settle down, particularly and it will be helped if you wear a compression garment over the area. If the area remains after six months or more, then minor liposuction to the area will help.   « less

This is a lot of surgery, but it can be done at one time, particularly if the operative time can be kept at a reasonable level. Proper precaution should be taken by your surgeons to avoid the serious complication of blood clots in your leg or pelvic veins that can lead to pulmonary embolism. Discuss this with your surgeons to make sure they are comfortable with the extent of surgery you are asking for.   « less

A full tummy tuck will remove a substantial amount of lower abdominal skin and fat and is often performed with liposuction. Clearly there will ultimately be some weight loss after all swelling has subsided and full healing has occurred. The amount of weight lost will be dependent on your current size and the volume of tissue that is removed. However, the procedure is designed to affect your contour and appearance, rather that the dial on your weight scale.   « less

I will answer this bluntly - First, I unaware of any insurance that will cover liposuction for cosmetic purposes. Second, if an insurance carrier did, by some miracle, cover liposuction, I doubt that you could find a competent surgeon to peform the procedure because of the scant reinbursement allowances available for covered procedures.   « less
  8. Dr. Rosenberg, I had full abdominoplasty done approx. 6mos ago to correct sagging skin and loose muscle from have twins, and would now like to have my belly button pierced. I went to a piercing establishment today and explained to them that I had a "new" belly button due to surgery, and they told me that they could not pierce people that have had abdominoplasty because the ring would grow out of the skin and leave a nasty scar. They explained that this is because the plastic surgeons make the skin so tight. I was a size 4 before I had children (size 8 now) and remember that my skin was tighter then than now and I had no problems then with my piercing. Is it true that a person cannot get a piercing after this surgery? - Nikki from West Virginia
This is nonsense. There should be no problem with re-piercing.

  9. Dear Sir, I am a 25-year-old woman and I'm not very happy with my calves. The lump that you usually see when contracting the calf muscles or wearing high heels is very large. I would like to know if there is a procedure for reducing this so that the muscles look normal (I can assure you it's not due to exercise!) - Mayita from United Kingdom

  10. hi im 36yrs old from Belfast Northern Ireland and i want to go and have my breasts lifted and possibly made bigger using implants but im not happy about the thought of all the scarring from my nipple down someone told me int the States u can do a reduction or lift by removing skin and tissue from the surrounding area of the nipple and tighten it up that way, is there any truth in that, thanks - lizzy from Belfast

Lizzy, A breast lift with an augmentation is a common procedure. A lift is basically accomplished by removing excess skin so as to tighten the skin "envelope" and lift the breast. Adding an implant will take out some of the skin slack and reduce the amount of skin resection needed for the lift. The scar pattern that is used for the lift will then depend on the degree is sag you current have. Sometimes this can be done so that the only scar you have is a circlular one around the nipple areola. For more severe sagging, an anchor type scar is necessary to get the best shape.   « less
