  1. I am considering breast reduction from a DD-cup to a C-sup. I have back problems and skin inflammation because of my breast size. Insurance will pay for this procedure, wont they? Is it best to couple the reduction surgery and lipo at the same time? - Anonymous from San Jose, CA

Many insurance companies will cover breast redcution if there is documentation by your physician that the procedure is not cosmetic. They usually require pre-op photos, and information about your height and weight along with a letter estimating the weight of the tissue to be removed. Most will pre-authorize with this information, so you know ahead of time if the procedure will be covered. Liposuction of the breast area is commonly done at the same time. Liposuction of other areas can certainly be done at the same time if your surgeon feels it is safe to do. It will depend on the extent of additional surgery and the length of additional operative time.   « less
  2. Hi, my 6 year old daugther got a scar from a skating accident, the scar is 3 month old. It is healing, but it is clearly going to be visible. Is there any creame that we can use to reduce the scar, I heard about mederma, scarfade and scarguard. Are these any good? any particular one is more recommended then others? - Greg Elkinbard from Belmont, Ca

There are a number of products that are potentially useful for topicall reduction os scars. The one that has been documented to work is application of silicone gel sheeting. The problem is that the materal must be placed on the scar a left in place, which will be difficult if near impossible for a six year old. The other products you mentioned are worth a try. Topical vitamin E has been shown to be ineffective.   « less
  3. Dr. Rosenberg, Three to four years ago, I had breast augmentation surgery (to at 36B), but I mistakenly used a scar lightner around the arerolas without thinking. So the borders of the arerolas look uneven, pigment lost. What would you recommend I do to "hide" the whitish scar and bring back the color? Tatooing? My last question is that my right breast hardened (capsular contracture). What can I do about that without spending much money? Thank you for your advice. I appreciate it! - Jenny from San Jose, CA

It is not clear whether the white scar is a result of the use of your lightening treatment or from just natural maturation of the surgical scar. I doubt if any topical treatment will work. Tattooing or surgical scar revision are you best alternatives. As far as the capsule contracture, manual compression of the capsule can be done to tear the capsule and soften the breast, bu tthis risks implant rupture and softeness lasts only several months until the capsule recontracts. Surgery is probably you best alternative for both the scar and the capasule contracture.   « less
  4. I had Liposuction done on my lower Abdomen and the sides 2 weeks ago. Now I have this tremendous, hard rounded, cone shaped lump- in place of the "fat-buldge" I had liposuctioned. IS THIS NORMAL? If so, WHEN WILL IT GO AWAY? WHAT IS IT? - T from las vegas NV

It is very common to have small lumps present after liposuction. Tunnels are created in the fat that soon fill with serum or blood. This is then replaced by scar tissue. Ultimately the scar tissue matures and virtually disappears. The large lump you have may represent a blood clot (not dangerous) or accumulation of fluid (a seroma). Obviously you need to check with your surgeon.   « less

Jenny, The simple answer is that elective surgery can be performed as long as: 1. your cardiologist agrees that the procedure can be performed with minimal risk and that 2. medication that might complicate the procedure (i.e. blood thinners) can be discontinued for an appropriate period of thime before and after the surgery. Most cardiac medication can and should be continued before and after the surgery.   « less

Saline injections?? Saline is effectively salt water and if injected the enhancement produced will last perhaps two hours! Maybe you mean silicone injections. This is also a terrible idea. Liquid silicone gel is known to migrate and who knows where it will end up over time. Fat injections (your own) do work if performed properly and the fat survives. If successful, the results are permanent.   « less
  8. Dr Rosenberg, I am a women in my mid fifties and I had a suture lift of the jaw line to correct my jowls. A small incision was made behind the ears and the underneath pulled back but I have gathering along the jaw line. I have been told not to worry as this will settle in time into a fold in front of the ear and not look strange but I feel unsure that this will happen and I will never be able to wear my hair behind my ears again without people knowing I have had a procedure. - Disey M

There has been a lot of promotion of the use of barbed threads to achieve a facelift using minimal incisions and a minimal procedure. The procedure is often done by physicians who are fully trained and certified in plastic surgery, although some plastic surgeons are offering this to patients. the sutures are expensive and the charges for this procedure, while not as high as a real facelift, are still substantial. At a recent conference of real plastic surgeons, it was revealed than many surgeons with significant experience with these threads are now using them as an adjunct at the time of real facelifts, and not as isolated procedures. The use of these threads basically amounts to "bunching" of the facial skin which is what you are experiencing. Risk also include visible threads seen through the skin, limited results, and short lived results. You represent one of these complications and if the problem persists, have the thread removed. Buyer beware!   « less
  9. I had a tummy tuck 3 years ago and the results were great, however, I am not crazy about the scar left in my belly button, I am considering having it pierced to help hide the scar. Is this an option or are there risks that I am unaware of. My belly button is still numb from the tuck, thats another reason I am considering it. I might not have to feel the pain!! - Anonymous

If you are considering breast reduction surgery, then the amount of tissue to resect will depend on your desired new size and the technical ability to achieve an appropriate new shape (this in turn will depend on your current breast shape and your height, weight and trunk size). Your local plastic surgeons can help you decide on an appropriate size.   « less
