  1. I had a breast augmentation almost 19 years ago. The implants were placed under the muscle. I had only gone up to a B cup at that time. I am now interested in getting a bigger size and having the implants placed over the muscle. Can this be done after so many years of the implant being under the muscle? Can it be done successfully?     Your implants can be replaced with larger implants. I would recommend that if the implants are replaced, they continue to be located behind the pectoralis muscles, but in a pocket that has been modified to create a partial submuscular position (or so-called dual plane position) to achieve the best result. Placement of your implants in a completely subglandular position (i.e. completely above the muscle) will likely not produce the best result.   « less

    Fat harvested from other areas of the body to place in the buttocks to enhanse the buttocks has become more and more popular. It requires that you have sufficient fat elsewhere to obtain for buttocks injection to achieve the result you desire. The biggest problem would be limited results of the procedure if sufficient injected fat does not survive after it is transplanted.   « less

    Computer analysis of the potential results of plastic surgery is a frequent tool used at the time of consultation with a plastic surgeon. I find it particularly helpful when discussing rhinoplasty (nose jobs), but it can also be performed with other procedures. The accuracy of the results depends on the skill of the individual doing the analysis, the sophistication of the software (often a variant of Photoshop), and the nature of the area to be analyzed. It is often helpful, but should not be considered to exactly duplicate the results of surgery.   « less
  4. 9 wks ago I had a breast lift and augmentation while the left breast is fine, the right one does not look normal. The silicone implant has not dropped down and sits high up on my chest wall. My dr. does not know why this has happened (he is board certified etc.) and now agrees that I will need some additional surgery. He says that he can do the work in his office under local anesthesia and that he would use the same implant that was inserted during the original operation. Is this a safe way to correct the problem? Could I get infected by reusing the original implant? Should I go to another surgeon to have this correction done? This doctor has a good reputation, but it bothers me that he doesn't know why this happened so I am nervous in having him "fix" the problem.     Adrienne, There could be several reasons why the right iplant is higher than the left. The most common would be some increase in scar tissue on the right side forcing the implant higher. Sometimes a minor internal bleed will produce this result. Correction of the proble could be as simple as expansion of the pocket downward to match the other side. There is no problem with using the original implant. Since your physician is the best one to understand how the procedure was performed, Iwould strongly recommend returning to him for what will probably be a minor revision.   « less

    Fat injections can be used to create fullness and lift to the buttocks with or without the use of implants. A significant amount of fat is required and this is obtained by liposuction from other parts of your body (abdomen, hips, back and thighs). If the injected fat survives, then the result is permanent. Injection of foreign substances like liquid silicone gel is illegal and will lead to disastrous problems.   « less

    Otoplasty for correction prominent ears can be performed in children under local anesthesia. Typically these kids are very motivated and are willing to put up with the discomfort of the local anesthesia injections. The procedure itself is painless. The combination of a caring, patient, and understanding physician along with a motivated and mildly sedated child can be successful. If these factors are not present, then general anesthesia in an excellent outpatient center is a safe alternative.   « less

    There are a number of techniques to successfully thicken lips. The simplest approach is the injection of a filler such as collagen or Restylane. This provides immediate results, but these fillers willlast only four to six months before they are reabsorbed and require repeat treatments. Surgically implanting strips of dermis (the structural substance of the skin) or artifical materials (like Softform) also work and are longer lasting, but can sometimes be felt. The best technique that I use is the injection of microdroplets of the patient's own fat obtained by performing a "mini-liposuction." Once the initial swelling is down, and with proper technique, the fat will provide a permanent soft result. This procedure is easily done under local anesthesia in the office.   « less
  9. I am almost 4 months post op from upper bleth surgery. 3 months ago, I had a raised whitish scar (hypertrophic) develop in the inner corner of one of my eyes where the incision was made. It extends up to the crease. I have been massaging it (p.s. advice) with no improvement. In fact, it looks like it has gotten bigger. ' The other eye is healing perfectly.   10. I am condiering having A tummy tuck with liposuction. After having my second child 6 months ago( c-section), I still have about 30 pounds to go, I went for a consult(Certified DR, who comes from many recommendations and is Chief of his hospital)and he says that I am ready to have the procedure now.     IF you believe that you will loose another 30 pounds before the surgery, I would advise you to wait. Weight reduction will reduce your intra-abdominal fat and allow the internal tightening of your abdominal fascia at the time of a tummy tuck to be more successful.   « less
